| A team of Physicians and Nurse Practitioners practicing in the long-term care continuum. Our Mission… To be the premiere provider of geriatric services in the long term care continuum. What is GAA? Geriatric Associates of America, P.A. (GAA) was formed by Dr. Keith Rapp in 1996 to provide quality Medical Directors, Attending Physicians and Nurse Practitioners to the long term care environment. Our association of over 40 Certified Medical Directors, Fellow-trained Geriatricians and Masters prepared Nurse Practitioners is one of the largest geriatric-based practices in the country. Our practitioners attend to patients at over 100 nursing entities and are Medical Director at over 65 of those in the Houston, Austin, San Antonio, Dallas and surrounding areas. The Long Term Care Environment… GAA is committed to the mission of enhancing the care of residents in the long term care environment. This commitment along with the ongoing support and training we provide our practitioners and the facilities they practice in has helped us to establish a solid reputation. We maintain a consistent presence in the facilities we serve so we are readily available to meet our patient’s and staff’s needs. What is our philosophy? GAA focuses on primary preventive care to help the individual stay well and, to the extent possible, avoid hospitalization. Our goal is to work with facility staff to provide medical care that will enable our patients to improve or maintain their highest level of physical and mental functioning. What services do we provide? Medical Services GAA provides clinical care through a staff of Attending Physicians and Nurse Practitioners. A unique aspect of our practice is the availability of resources that support and enhance the care given to our patients that only a large practice experienced in long term care can provide. Physician involvement is crucial due to the increasingly complex patients now being admitted to long term care facilities. Patients have the right to choose a physician who will actively supervise all aspects of their medical care and regularly visit them at the facility. Medical Direction All of our physicians are Certified Medical Directors (CMD) through the American Medical Directors Association (AMDA), or are in the process of becoming certified. The American Medical Directors Association is the professional association of medical directors and physicians practicing in the long term care continuum dedicated to excellence in patient care by providing education, advocacy, information, and professional development. Our physicians are knowledgeable about the clinical aspects of management of the frail older adult and federal and state guidelines regulating long term care. Our medical management model strives to provide quality care and increase regulatory compliance. We have also developed over 30 educational presentations to provide as a service to our facilities for staff development. Integrated Nurse Practitioner Model This is a unique model of care created by GAA devoted not only to caring for our residents, but also providing administrative assistance to the facility. We look for our Nurse Practitioners to become an integral part of the facility team. Responsibilities include participation in interdisciplinary team meetings, formal and informal staff education, the ability to evaluate all residents exhibiting significant changes in condition, as well as gathering and making recommendations on quality indicator data. Our model has benefited facilities in such areas as decreased weight loss, restraints, number of medications and hospitalization rates. In addition, this model has shown to be successful in increasing patient and family satisfaction and decreasing medical liability risk for the facility. Consulting Services Our experienced administrative and clinical staff is available to other long term care practices and entities to provide a variety of presentations including Medical Direction, Billing and Coding and numerous clinical topics. We are also available to provide consulting services to nursing facilities across the country to offer feedback and assistance regarding quality assurance and systems implementation. |
